services. This session will generate thought-provoking
conversation about how we want to live out our final
days and will provide information about care choices,
as well as provide guidance on how to respond to patient
and family needs.

need to put our end of life wishes down in writing –
and make sure our loved ones know what we would want.
This workshop will provide an opportunity to learn about
end of life choices, and will review legal procedures,
forms and documents to help ensure your wishes are followed.
Resources to assist in this process will also be presented. |
